Overview
The FDS Form 1 is for M-NCPPC employees holding positions which must complete Financial Disclosure Statements (FDS), pursuant to Administrative Practice 2-24, Code of Ethics. Please carefully follow the FDS Instructions below and complete FDS Form 1. Additional copies of the FDS schedules have been provided below, if needed. Note: To sign the document electronically, ensure that you save it to your computer and open the document with Adobe Acrobat. The form may not be able to be electronically signed when opened with a web browser.
