All forms must be signed by the member, benefit recipient or authorized representative. Authorized Representatives should call the ERS Office for direction on submitting proper legal documentation. In addition, some forms require a signature from a spouse, witness or Notary Public. Please be sure all necessary signatures are included.
Double-check that all information and any supporting documentation is provided. Incomplete forms will be returned, causing a delay in your request. Mail form to:
